Subject: Application for Review and Removal of Wrongful Blacklisting of Passport
Respected Sir,
I, ______________________ S/o ______________________, CNIC No. ______________________, resident of ________________________________, most respectfully submit this application for kind consideration of the Review Committee.
That I am a Pakistani citizen and holder of Passport No. ______________________, issued on __________ and valid up to __________.
That to my utter shock and dismay, I recently came to know that my passport has been blacklisted in your esteemed department’s record, due to which I am unable to travel abroad. I submit that the said blacklisting has been done wrongly, erroneously, and without any prior notice, show-cause, or opportunity of hearing afforded to me.
That I have never been involved in any criminal case, inquiry, or proceedings before any court or authority, nor have I violated any law that could justify such action. The impugned blacklisting is therefore unjust, unlawful, and violative of the principles of natural justice as well as my fundamental rights guaranteed under the Constitution of the Islamic Republic of Pakistan.
That due to the wrongful blacklisting of my passport, I am facing severe hardship, mental agony, and financial loss, as my lawful travel for employment/business/education/personal reasons has been restrained.

In view of the above facts and circumstances, it is most humbly requested that this application may kindly be placed before the competent Review Committee, my case be reviewed at the earliest, and my name may graciously be removed from the blacklist, thereby restoring my passport for normal use.
Any other relief deemed just and proper in the circumstances may also kindly be granted.
